Junk Removal Service with Payment Authorization

Part 1: Junk Removal Service Terms

I, the Customer requesting service, agree to the following terms for junk removal services provided by Dirty Boyz Waste Solutions.

Pricing & Payments:
  1. Pricing based on volume (space in truck), labor, weight, and material type.
  2. Standard load pricing tiers:
    • ½ load: $469 – includes up to 1 hour of labor and 0.75 tons
    • ¾ load: $649 – includes up to 1.5 hours of labor and 1.125 tons
    • Full Load: $799 – includes up to 2 hours of labor and 1.5 tons
    • Smaller Pickups: Custom quoted based on item type, labor, and distance. Minimum charge applies.
  3. Additional charges (if applicable):
    • Extra labor: $99.00/hour per 2-man crew (beyond included time)
    • Overweight disposal: $120.00/ton over included weight (pro-rated)
    • Elevator/stair carry: $49.00
    • Disassembly/powertool usage: $49.00 starting
    • After-hours scheduling: 20% surcharge
  4. Cancellation & job site charges:
    • Same-day cancellation within 6 hours: $49.00 non-refundable
    • Dry load: $99.00 (crew arrives, no items available)
  5. Additional items not in original quote charged upon completion.
  6. Customer responsible for landfill fees for hazardous materials.
  7. $50 deposit required to confirm. Balance due upon completion. Deposit refundable if cancelled within 24 hours. 10% late fee after 10 days past due.
Access & Property Conditions:
  1. Customer ensures safe, legal, unimpeded access. Delays may result in wait time fees.
  2. Not liable for property damage during reasonable removal unless due to clear negligence. Customer assumes liability for approved contents.
Part 2: Credit Card Authorization

By submitting this form, you authorize Dirty Boyz Waste Solutions to charge the credit card provided for agreed upon junk removal services. Your information will be saved to file for future transactions on your account, according to our security practices (we do not store full card numbers).
